KARADENİZ TECHNICAL UNIVERSITY FACULTY OF ENGINEERING DEPARTMENT OF SOFTWARE ENGINEERING INTERNSHIP APPLICATION DOCUMENTS

1- Internship Application Form: The Internship Application Form can be downloaded in PDF format via the link provided on the relevant page and filled out digitally. Alternatively, a Word version of the same form can also be downloaded from the specified address.

After completing the sections related to themselves, students must send the form to the institution where the internship will be carried out and request the relevant sections to be filled in by the institution. The form must include the student?s signature as well as the signature and official stamp of the authorized representative of the institution. Documents missing a signature or stamp will not be accepted under any circumstances.

Once the internship application is approved, the "Internship Commission Approval" section of the form will be signed by the Chair of the Internship Commission. Students who have a new national ID card do not need to fill in the sections related to old identity information.

The number of working days between the internship start and end dates must be calculated in accordance with the rules specified in the Internship Regulations and written on the form.

2- Family Health Insurance Declaration Form: This document can be downloaded and filled out from this link. It is not required for foreign students.

3- Health Insurance Eligibility Certificate: This document is obtained via the "SPAS Eligibility Inquiry (Health Insurance Activation System)" service of the Social Security Institution on the e-Government Portal. The barcode document must be generated and submitted as a PDF file. Students who cannot obtain eligibility due to debt or similar reasons must submit a readable screenshot of the relevant web page. This document is not required for foreign students.

4- Internship Application and Occupational Safety Petition: This document can be downloaded and filled out from this link.

5- Workplace Internship Agreement: This document can be downloaded and filled out from this link. The agreement must be signed by the student, the company, and the Dean's Office. The student is responsible for submitting the document to the Dean's Office for signature. If the internship is unpaid and the company does not require an agreement, this document is not mandatory.

NOTE: IF A STUDENT MEETS THE REQUIREMENTS FOR DOING AN INTERNSHIP DURING THE SEMESTER AND WILL CARRY OUT THE INTERNSHIP WITHIN THE SEMESTER, THE PETITION BELOW MUST BE SUBMITTED. THIS PETITION IS NOT REQUIRED FOR SUMMER INTERNSHIPS.

6- Internship Application Petition for Semester Internship: This document can be downloaded and filled out from this link. It must be completed by students who meet the requirements for doing an internship during the semester as specified in the Internship Regulations. Students who will complete their internship during the summer term do not need to submit this document.
